Below are the documentation items my team will need in order to begin processing your loan. Once your processor has reviewed the loan application and documentation, he/she could ask for more documentation as needed. Our goal is to obtain a speedy underwriting approval, so your prompt attention to these items is greatly appreciated!

All documents may be submitted in one of the following ways:

  • Email to me, Katy McCoy (; Katy is my Loan Officer Assistant) or to your processor (once assigned)
  • via the online application tool (return to APPLY NOW, log in and upload documents)
  • via PointServ (applicable to items marked with an asterisk *)
  • via Fax: 866.424.1825
  • another secure document transfer method that you are comfortable with


  1. W2s for the past 2 years*
  2. Pay stubs covering a one month or greater period*
  3. Tax Returns for the past 2 years* (if you are self-employed, both business and personal returns)
  4. 1099s
  5. K1s
  6. YTD Profit and Loss Statement and Balance Sheet for any businesses that you own (i.e. consulting business)
  7. Bank Statements: checking, savings, 401k or other retirement savings, investment, etc. covering a continuous 60 day period*
  8. Explanation and documentation of any non-payroll deposits into your bank accounts (we may ask for further details later in the process)
  9. Name and phone number of the insurance company or agent who will be handling the home owners insurance for your new home
  10. Copy of driver’s license
  11. Copy of Social Security card
  12. Your landlord’s contact information, if you have rented your home during the last 2 years


  1. Property taxes for current properties owned
  2. A copy of your current mortgage statement
  3. A copy of your latest Home Equity Loan or Home Equity Line of Credit statement (if applicable)
  4. A copy of the HUD 1 from your purchase of your home (refinances only)
  5. A copy of your existing survey if it accurate (refinances only)


  1. Copies of your past two years’ tax returns (with all schedules, including K-1s and/or 1099s if applicable);
  2. Copy of current profit and loss statement and balance sheet;
  3. Copy of corporate/partnership tax returns for last two years if owning 25% or more of company plus copies of W-2s and/or 1099 forms.


  1. Relocation agreement if move is financed by employer (i.e. buyout agreement plus documentation outlining company-paid closing costs benefits)
  2. If you’ve had a previous bankruptcy, bring copies of petition for bankruptcy and discharge, including supporting schedules
  3. Divorce decree (if applicable)
  4. Documentation supporting monies received from Social Security/retirement trust income, i.e. copies of direct deposit bank statements, awards letter or evidence income will continue


  1. FHA: Copy of Social Security Card and driver’s license for each applicant and co-applicants
  2. VA: Original certificate of eligibility and copy of DD214 discharge paper
  3. VA: Name and address of nearest living relative
